An HPLC method coupled with UV detection and electrospray ionization mass spectrometry, HPLC–UV–ESI-MS, was developed for determination of sildenafil 82, vardenafil 98, and tadalafil 99 in dietary supplements. The separation was achieved on a C18 column with acetonitrile and an aqueous solution containing 20 mmol NH4OAc and 0.2% formic acid as mobile phase. The standard deviation varied from 2.0 to 5.6 % <2005JCH89>. A method is described for extraction and determination of sildenafil citrate and its metabolites in human serum by micellar electrokinetic capillary chromatography (MEKC) coupled with sample stacking and polarity switching <2002JCH279>. Liquid chromatography–electrospray ionization mass spectrometry (LC–ESI-MS) for analysis of sildenafil has also been developed and applied to 40 botanical products.

[22] It then became the most widely used drug for the treatment of all forms of sildenafil syrup motion sickness. There are other popular examples of drugs derived from clinical observations. Binding Characteristics
Nitric oxide is a second messenger molecule that turns on (pun intended) the enzyme guanylate cyclase, which converts guanosine triphosphate to cGMP. The vasodilator cGMP relaxes the smooth muscle in the corpus cavernosum, allowing blood to flow into the penis, thereby producing an erection. However, phosphodiesterase-5 (PDE-5) hydrolyzes the cGMP, which causes vasoconstriction and the outflow of blood from the penis. The level of vardenafil found in the Chinese formulation might induce an overdosage in patients. In the case of vardenafil, there was no evidence of a dose-dependent improvement in efficacy beyond the 10 mg dose but problems of drug–drug interactions may occur. For example, vardenafil is contraindicated with nitrates as it potentiates their hypotensive effect. Also vardenafil (10 or 20 mg) when concomitantly administered with α-blockers (such as terazosin and tamsulosin) and with nifedidine to healthy volunteers resulted in some subjects experiencing hypotension.30 The presence of homosildenafil (see Figure 2) has already been described in functional food marketed for penile erectile dysfunction31 and in dietary supplements32 but toxicological data are not known or not available. Thus, due to possible side effects or drug–drug interactions, a significant risk is faced by consumers who purchase drugs marketed for erectile dysfunction via the Internet.

Pfizer has redesigned the synthesis of several of its pharmaceutical products to reduce generation of hazardous waste. Changes were made in the synthetic route to sildenafil citrate (see Fig. 9.7), the active ingredient in Viagra® (Dunn et al., 2004), which resulted in a more efficient process that required no extraction and solvent recovery steps (see Fig. The E-factor (Sheldon, 1992) for the process is 6 kg waste/kg product, which is substantially lower than an E-factor of 25–100, which is typical of pharmaceutical processes. Furthermore, all chlorinated solvents had been eliminated from the commercial process.

During the medicinal chemistry stage in 1990, the solvent usage was 1816 L/kg, and the optimized process used 139 L/kg solvent, which was reduced to 31 L/kg during commercial production in 1997 and to 10 L/kg with solvent recoveries. Pfizer plans to replace t-butanol/t-butoxide cyclization with an ethanol/ethoxide cyclization. Combined with other proposed improvements, this is expected to increase the overall yield from 76–80% and further reduce solvent usage and organic waste. High-performance liquid chromatography (HPLC) <2005JCH118> and HPLC–MS <2005JCH89> were developed for determination of sildenafil in biological fluids. Reverse-phase high-performance liquid chromatography (RP-HPLC) has been used for quantitative analysis of sildenafil citrate in pure form and its pharmaceutical formulations <2002MI743>. its release in 2004 for all of its indications [32].
Mechanism of action
Sildenafil (1; Figure 1), also known as Viagra®, is one of the most notable drugs that has been repositioned. It is a phosphodiesterase (PDE) type 5 inhibitor that was originally developed to treat angina, but it was repositioned during clinical testing to treat male erectile dysfunction in order to fully take advantage of its therapeutically relevant side effect profile [15]. As a result of this serendipitous repositioning, sildenafil citrate has grossed over $15 billion in revenue since its release in 1998 [16]. Another notable example is thalidomide (2), a sedative that received much notoriety in the early 1960s due to its severe teratogenic effects [17]. Despite its infamy, thalidomide was repositioned to treat erythema nodosum leprosum and was approved for the treatment of this form of leprosy by the Food and Drug Administration (FDA) in 1998 (Thalomid®) with the caveat of its teratogenic effects [18].

Moreover, thalidomide was repositioned again to treat multiple myeloma and FDA approved in 2006 in combination with dexamethasone for newly diagnosed multiple myeloma patients [19]. Some recent examples of drug repositioning include aztreonam (3) and duloxetine (4). Aztreonam is a monobactam antibiotic that was approved in 1986 for the treatment of Gram-negative bacterial infections via intravenous or intramuscular injection (Azactam®) [20]. This drug was marketed to treat bacterial infections and is safe for patients with penicillin allergies [21]. Previously, aztreonam was only approved for intravenous or intramuscular injection but not for inhalation because the solution for aztreonam contains an arginine buffer.
Dosage and Administration
Arginine is a substrate for nitric oxide production in many organs, and the generation of nitric oxide can lead to tissue injury [22]. Previous inhalation studies in patients with cystic fibrosis demonstrated that long-term use of inhaled arginine was associated with airway inflammation and deterioration of symptoms [23]. Aztreonam was later repositioned as Cayston® for inhalation by reformulating the solution to contain lysine rather than arginine [24]. Duloxetine is a serotonin and norepinephrine reuptake inhibitor which is marketed for major depressive disorder (Cymbalta®) in the United States and for stress urinary incontinence (Yentreve®) in Europe [1]. Trial studies revealed that duloxetine reduced painful physical symptoms in depressed patients [26], which led to its FDA approval for the management of diabetic peripheral neuropathic pain (DPNP) in 2004 [27] and fibromyalgia in 2008 [28].
